オンラインカジノでクレジットカードで入金してから勝って出金する場合、または新しいカードを使ったときなど、サポートから「記入して送れ」ということで、fax back formとかcredit card verification formとかpurchase authorization formというような書式が送られてくることがあります。
最近ではNETELLER(ネッテラー)やClick2payで入金した場合でも、こうした書類の提出を求められることがあります。
この書類の目的は、そのプレイヤーが確かにそのカジノで自分の意思で入金をしたという証拠を残すことです。主にクレジットカードのチャージバックを防ぐのが狙いです。
やっかいなことに大抵の場合はフォームは英語なので、何を書けばいいのか分からないという方々から頻繁に質問をいただきます。これまで個別に回答してきましたが、どこのカジノでもだいたい同じ様式なので、代表例として二つ種類のFormの書き方をご紹介します。いずれもRTG系のオンラインカジノです。
なお、言うまでもないですが、名前や住所などはそのカジノに登録してある通りに書いてください。
